Disability Support

Social Work Therapy for NDIS participants

Grief Connect® provides Social Work Therapy for self-managed and plan-managed NDIS participants, where the support relates to the participant’s disability, functional needs and individual goals.

The focus is on building capacity, strengthening skills and supporting greater independence in everyday life.

Disability, grief and significant life change

People living with disability can also experience grief, bereavement and adjustment to significant life change. Where these are affecting a participant’s disability-related functioning, Grief Connect® can provide Social Work Therapy focused on practical skills, strategies and capacity to support everyday life, relationships and community participation. Support is guided by the participant’s goals, needs and circumstances.

Social Work Therapy may include

  • Building skills and strategies for everyday functioning
  • Supporting emotional regulation and adjustment to significant change, where these affect disability-related functioning
  • Building communication and interpersonal skills
  • Developing strategies to increase independence and participation in family, social and community life
  • Working with parents, carers and other supports where this contributes to the participant’s goals, and liaising with other professionals and services with the participant’s consent
  • Referral to mainstream, health or community services where needs fall outside the scope of NDIS-funded Social Work Therapy

Access and funding

Grief Connect® currently works with self-managed and plan-managed NDIS participants, with no gap fee. NDIS-funded services are not currently available for NDIA-managed participants. Services are delivered online, Australia-wide.

Access to NDIS funding will depend on the participant’s individual plan, goals and funding arrangements, so participants are encouraged to confirm that Social Work Therapy can be funded through their plan before commencing. Grief Connect® will discuss goals, support needs and funding arrangements first, to determine whether the service is appropriate and within the scope of the Social Work services offered.

Grief Connect® can provide Social Work support, practical guidance and education for parents, carers and support networks, to strengthen their capacity to support the participant’s disability-related needs, independence and goals. This support may be provided where it directly relates to the participant’s NDIS goals. Every support package is shaped around the individual rather than a fixed programme.
